Sealed bids will be received by
the Early County Board of
Commissioners, Blakely, Georgia
at the Office of Early County
Administrator, Spencer Mueller,
204 Court Square, Blakely,
Georgia 39823 on October 2,
2017 until 4:00 PM Local Time
and at that hour opened and publicly read aloud for the improvements to the Airport as listed herein. Please note that bids will only
be considered by those bidders and
subcontractors currently pre-qualified with the Georgia Department
of Transportation.
PROJECT DESCRIPTION
The work consists of furnishing
all labor, equipment, and materials and performing all work in
strict accordance with the plans
and specifications for:
CRACK SEAL & REMARK
AIRFIELD PAVEMENT
The location of the work is at
the Early County Airport, Blakely,
Georgia.
Prospective bidders should read
the following instructions carefully before submitting their bids.
For each item on the bid form
there is a space provided for the
price to be shown in numerals and
words. All notations must be in
ink. Totals read at the opening of
bids are not guaranteed to be correct and no final award of contract
will be made until the bid and
extensions have been verified.
A Bidder’s bond must be executed on the form furnished by the
Owner, and the required bond,
cash, cashier’s check, or certified
check must accompany each proposal, in the amount of 5% of the
total amount of the proposal. A
100% performance bond and a
100% payment bond will be
required of the Contractor at time
of contract execution. A Georgia
Resident Agent must countersign
all bonds from a surety company
authorized by law to do business
in this State pursuant to a current
certificate of authority to transact
surety business by the
Commissioner of Insurance; no
bond shall be approved unless the
surety is on the United States
Department of Treasury’s list of
approved bond sureties.
The successful bidder will be
required to provide the Owner
with the affidavit required by
OCGA 36-91-21 (e) Competitive
Award Requirements.
All work under the contract
shall be completed within Thirty
(30) calendar days from the
issuance of the notice to proceed.
Liquidated damages for delays
in completion will be in accordance
with the following schedule:
Original Contract Amount $0-
$49,999, Daily Charge $200;
Original Contract Amount
$50,000-$199,999, Daily Charge
$350; Original Contract Amount
$200,000 and over, Daily Charge
$500.
Payment will be made monthly
on completed work. Retainage will
be held by the Owner to a maximum of ten percent (10%) of each
progress payment.

DBE OBLIGATION. The bidder shall make good faith efforts,
as defined in Appendix A of 49
CFR Part 26, Regulations of the

Transportation, to subcontract 0%
percent of the dollar value of the
prime contract to small business
concerns owned and controlled by
socially and economically disadvantaged individuals (DBE). In
the event that the bidder for this
solicitation qualifies as a DBE, the

will be considered non-responsive.
Contractor and Subcontractor
must state affirmatively that the
firm has registered with and is
participating in a federal work
authorization program in accordance with the applicability provisions and deadlines established in
O.C.G.A. 13-10-91.
Title VI Solicitation Notice:

Commissioners, in accordance
with the provisions of Title VI of

appointed and qualified Board of
Commissioners of Early County,
Georgia, sitting for county purposes: IT IS ORDERED that One
Hundred (100.00) cents on the one
hundred dollars, or Ten
Thousands (10.00) mills on the
dollar of taxable property in the
incorporated areas of Early
County, Georgia, as shown by the
Tax Digest of said county for the
year 2017, be, and the same is
hereby, levied, and it is ordered
that the same be collected by the
Tax Commissioner of Early
County, Georgia, ad valorem, upon
all of the property subject to taxation in said incorporated areas in
said county for county purposes.
FURTHER ORDERED that
One Hundred (100.00) cents on the
one hundred dollars, (10.00) mills
on the dollar of taxable property in
the unincorporated areas of Early
County, Georgia, as shown by the
Tax Digest of said county for the
year 2017, be, and the same is
hereby, levied, and it is ordered
that the same be collected by the
Tax Commissioner of Early
County, Georgia, ad valorem, upon
all of the property subject to taxation in said unincorporated areas

These figures reflect the rollback
of the unincorporated area millage
to account for the insurance premium amount collected, said rollback being in the amount of Zero
(0.00) mills. Of the $248,300 total
insurance premium tax received
by Early County for 2017,
$248,300 is allocated to provide
fire protection service to the unincorporated areas of the county and
$0.00 is used to rollback the millage rate.
IN ADDITION to the above and
foregoing levies, upon the certification

of the Board of Education of
Early County, Georgia, there is
hereby levied upon all taxable
property of and in Early County,
Georgia, a tax of Fifteen & Seventy
Five Hundredths (15.75) mills on
the dollar of its valuation for tax
purposes, said levy being made for
the purpose of maintaining and
operating the public schools of
Early County, Georgia.
IT IS ORDERED that the above
and foregoing tax levies, in the
amounts and for the purposes stated, be made, and the same be collected by the Tax Commissioner of
Early County, Georgia, for the year
2017.
IT IS FURTHER ORDERED
that a copy of this tax levy be
advertised for thirty (30) days at
the County Courthouse of Early
County, Georgia, in Blakely,
Georgia, and be published in the
Early County News, the official
organ of Early County, Georgia,
once a week for four weeks over a
period of thirty (30) days from the
date hereof, and that a certified
copy of this tax levy be furnished
to the Tax Commissioner of Early
County, Georgia.
FURTHER ORDERED that a
certified copy of this tax levy be
forwarded with the 2017 Early
County, Georgia, Tax Digest to the
State Revenue Commissioner of
the State of Georgia, as required
by law, for consideration by that
official toward approval of said Tax
Digest and this tax levy.

NOTICE TO THE PUBLIC OF
FINDING OF NO
SIGNIFICANT IMPACT
For Proposed Blakely Clinic
Expansion
In accordance with the National
Environmental Policy Act, the
Council on Environmental Quality
regulations for implementing
NEPA (40 CFR Parts 1500
through 1508) and the HHS
General Administration Manual
Part 30 Environmental Protection
(February 25, 2000), HRSA has
determined that the Blakely Clinic
Expansion proposed by PRIMARY
CARE OF SOUTHWEST GEORGIA, INC. will have no significant
adverse impact on the quality of
the human environment. As a
result of this FONSI, an
Environmental Impact Statement
will not be prepared.
The Health Resources and
Services Administration (HRSA)
of the Department of Health and
Human Services (HHS) provides
discretionary grant and cooperative agreement awards to support
health centers expand their capacity to provide primary and preventive health care services to medically underserved populations
nationwide.
PRIMARY CARE OF SOUTHWEST GEORGIA, INC. in Blakely,
Georgia has applied for HRSA
grant funding. The applicant proposes to use grant funds to construct an expansion of approximately 4,300 square feet at their
existing facility located at 360
College Street, Blakely, GA, 29695.
The project will also involve a
parking lot expansion of approximately 15,200 square feet. The
applicant has submitted an
Environmental Assessment (EA)
that documents impacts of the proposed action. This EA is incorporated herein by reference into this
FONSI.

is contained in the Environmental
Assessment for this project, which
is on file at the following address
